[Herkimer, NY] – January 13th, 2026 –Herkimer County HealthNet, in partnership with Bassett Medical Center Research Institute, is calling on residents of Herkimer and Dolgeville to participate in a Community Health Survey. The survey is designed to gather comprehensive, confidential information to assess the community’s health status and needs.
300 randomly selected households in Herkimer and Dolgeville, NY will receive a postcard with a unique QR code to access the survey. This innovative approach aims to increase participation, particularly among smartphone users, and reduce survey costs by encouraging online responses. Community members aged 18 and older who receive the postcard may also call to request a paper survey or to complete the survey by phone. Participants who complete the survey will receive a $15 gift card for their time.
The results will be used to develop and evaluate future diabetes prevention efforts. Questions cover a range of important topics, including access to healthcare, chronic disease prevention, mental health, and social factors that influence well-being.

Key Survey Details:
- What: 2026 Rural Impact Community Survey
- Who: Random sample of residents in the towns of Herkimer and Dolgeville aged 18 and older
- Where: The survey can be accessed online using a unique QR Code found on the postcard. Paper copies are also available by calling the number listed on the postcard.
- When: The survey will be open from January 20, 2026 to February 28, 2026.
- Why: Results will inform future community-based diabetes prevention efforts developed through the Rural Impact project, funded by the National Institute of Nursing Research of the National Institutes of Health under Award Number UG3NR022298.
